Health IT company RightCare, which developed a way to identify and track patients at risk for readmission, will start offering a content tool for patients as part of a collaboration with Emmi Solutions, according to a company statement. The deal is designed to help patients better understand their care plan and condition. The goal is to improve interactions between patients and providers to cut readmission risk.

RightCare will offer Emmi Solutions’ interactive technology with its own predictive analytics tool. The patient engagement content from Emmi will become RightCall to help providers and case managers anticipate potential readmissions. Questions for RigtCare’s Discharge Decision Support System are presented by nurses and discharge planners. If their responses trigger an alert, patients are referred for post-acute care support. Post-acute care could include long-term hospital care, rehabilitation care and care in a skilled nursing facility.

Emmi Solutions products include personalized calls and interactive content to help patients understand their treatment options. It steers away from complex language and uses images to more effectively reach patients. One objective of the software is to ensure patients get follow-up care and steers them to schedule follow-on appointments.

Earlier this year RightCare added a COO in the runup to the commercial launch of its predictive analytics platform.
