Tandigm Health and a group of physicians are collaborating to bring analytics tools and real-time data to a network of some 350 doctors.

Both Holy Redeemer Health System and 75 physicians from  its affiliated Innovative Wellness Alliance will work with Tandigm, based in Philadelphia and started last year as joint venture between Independence Blue Cross and DaVita Healthcare Partners. The effort was started to increase the number of primary care physicians in the region and to better manage chronic illnesses and reduce costs through collaboration and technology.

Tandigm will have a network of more than 350 primary care physicians. The deal marks Tandigm Health’s first cooperative arrangement with both a clinically integrated network of primary care physicians and a full-service health system.

“Enthusiasm for our model among primary care physicians in our region has exceeded our expectations,” said Dr. Anthony Coletta, president and CEO of Tandigm Health, in a statement.

The IWA was formed in January, 2014 as a clinically integrated network comprised of primary care physicians, nurse practitioners, and Holy Redeemer.
