A collaboration between Welltok and Centura Health tied to Welltok’s Café Well Concierge platform, powered by IBM Watson, wants to guide patients recovering from heart procedures or a heart attack to make healthy decisions as they go through the rehabilitation program.

The deal marks a major milestone for Welltok. Centura Health is the first health system to adopt the app.

Among the ways it supports patients are:

  • Responding to their questions about daily health management and recommendations;
  • Finding options for cardiac rehab exercises and activities based on things like geography and preferences;
  • Researching new heart-healthy recipes and dishes at local restaurants;
  • Identifying educational resources and videos on living with heart conditions
  • Participating in online communities and support groups

The app uses natural language processing to develop insights about its users as they work to meet health goals outlined in their rehabilitation. It makes suggestions to users based on evidence-based guidelines. Over time it is designed to produce options or suggestions and rewards that are more personalized.

Jon Gardner, vice president, Centura Health Heart and Vascular Network said: “Our physicians and health care teams are dedicated to an integrated approach that engages consumers with resources they can use beyond the clinical setting. We see this as the first step in delivering a digital health solution that meets health needs and rewards consumers for achieving personal health and wellness goals.”

Centura Health Heart and Vascular Network includes 15 hospitals, six senior living communities, health neighborhoods, physician practices and clinics, home care and hospice services.

In a phone interview last month, Welltok CEO Jeff Margolis said  its Café Well platform was attracting increasing interest from a diverse group of organizations.
