Taking center stage at the upcoming population health conference, MedCity INVEST Pop Health Virtual, will be healthcare startups seeking to address some of the unmet or inadequately met needs in healthcare. The arena of population health offers ample opportunities for healthcare entrepreneurs to prove themselves.

For the second year, MedCity News is partnering with the New Orleans Business Alliance to host the executive summit scheduled for November 16-18. It will spotlight conversations on themes related to population health from chronic conditions to social determinants of health. You can check out the agenda here.

We have a limited number of tickets for $20.20. If you act now and register using the code MCN2020 by November 14, you will be eligible to receive them. Click here to register.

As part of the Pitch Perfect event startups selected by MedCity News will present their vision on how their products and services may be used to improve overall population health followed by Q&A with judges. Startups will be asked about their business model, reimbursement and partnerships among other questions. The audience will vote on the startup that they believe will likely have the most impact, and the judges will select a winner that will be declared the 2020 INVEST Pop Health Leader. The event will be held on Wednesday, November 18 from 3:30-5pm ET.

Here are the startups addressing population health and chronic conditions that will take part in the contest:

Dina powers the future of home-based care. The AI-powered care coordination platform activates multiple home-based service providers, engages patients directly, and plugs into remote monitoring devices. The platform creates a virtual experience for the entire healthcare team so they can communicate with each other–and help patients and families stay connected–even though they may not physically be under the same roof. Dina helps professional and family caregivers capture rich data from the home, using artificial intelligence to recommend evidence-based, non-medical interventions.

Happy was created to ensure that every person, at every time of day and night, feels heard, understood, comforted, encouraged and important.

Pack Health delivers digital health coaching to manage 25+ chronic conditions, behavioral health, and social determinant of health needs. Connecting with members weekly via phone, text and email, the company’s engagement is highly personalized to deliver tailored education, accountability, and resource coordination based on each individual’s goals and barriers. Pack Health serves customers across health plan, employer, pharma, health system, and non-profit verticals.

Pair Team is an operational platform and care team to automate value-based population health. The platform monitors health plan and system data to trigger automated workflows that engage patients to schedule clinically impactful visits, surface care recommendations to clinicians, and manage follow-up care coordination. Their bolt-on model allows them to work as an extension of your care team within existing processes and accelerate quality programs in days, not months. For practices, this drastically improves care quality and visit efficiency. For plans, this aligns day-to-day operations with total cost of care. For patients, this improves health outcomes and relationships with their health providers.

Phox Health’s software-based platform connects pharmacies, virtual practices, and health systems to two-hour prescription delivery nationwide.

Pyx Health is a mobile solution that reduces loneliness and social isolation by connecting with a healthcare organization’s members outside of the traditional care setting. By providing critical and timely interventions and addressing social determinants of health, the company takes care of healthcare organizations’ members when they are most vulnerable, especially after a transition of care.

The judges include:

Ian Chiang, Principal, Flare Capital Partners
Branden Fini, Senior Associate, Providence Ventures
Robert Tucci, Managing Director, Texas Halo Fund
