The objective of patient and family centered care is to meliorate the quality of care provided in healthcare. This task is challenging, since resources available for quality care is always limited. With incessant increasing costs of healthcare, primary care provided to the patient is affected. The adoption of the Patient-Centered Medical Home model can be used to serve quality care to the patient in the shape of collaboration. In PCMH model, patients and their families are key components in providing individualized healthcare decision making for them.

Through patient centered care, the information is shared among patients, their family and friends. Care to share sensitive non-medical information is taken into consideration. In it, Patients’ needs and preferences are taken care of, in order to provide transparent service.

Through Patient-Centered Practice, doctors, nurses, and other staff members are engaged in systematical development of improving the care process for the patient. Plans are aimed towards educating the patients. They are molded in a way to let patients choose their own competence level. Thus making the implementation of the skills learned through education smoother. Patient centered care is focused on population health management programs which work directly with individuals at high risk for chronic illness. They promote healthy behaviors in individuals who are prone to diseases and assist them in managing their conditions, without the input of their physician.

PCMH model aims to strengthen the clinical and organizational leadership by the use of software technology like electronic medical records. It also endeavors to engage policymakers at three levels: local, regional and national level. The demographic of every region is catered to, and the data collected is aimed for improvement and research purposes.

According to one of the initiative, ‘Air Force’—a Patient-Centered Medical Home authorized by the State in 2011, was a success. With their PCMH model, they were able to reduce 14% emergency department (ED) and urgent care visits. A total of $300,000 was annually reduced through improved diabetes and care management. Apart from that, 77% of diabetic patients improved glycemic control at Hill Air Force Base.

The whole concept of Patient Medical Centered Medical Homes revolve around speedy care delivery to the patient. With around-the-clock telephone or electronic access (email) to a member of the care team, the medical home practice is responsive enough to meet the demands of the patients’ with urgency.

The target of patient centered care is to improve the care processes. It is a challenging task, since the procedures and hurdles faced in clinical decision making and practice management requires high level of competence. Research work done into analyzing data gathered through PCMH model will strengthen the future development of patient centric care model.
