Using SeeChange Health‘s wellness anaylitics platform over the past five years, JFK Health said it has saved $5 million while cutting both hospital admission and readmission rates significantly among its roughly 8,000 employees.

JFK Health, based in central New Jersey, has been partnering with San Francisco’s SeeChange on employee wellness as part of its overall efforts to curb costs on health benefits, which were increasing “with double-digit trends.”

In response, the health system identified disease management and prevention as key goals to reverse the trend. It lists the following metrics:

— 31 percent decrease in hospital admissions
— 50 percent decrease in hospital readmissions
— 48 percent higher cancer screening compliance than the national average
— 35 percent higher diabetes management than the national average
— $2.8 million in benefit savings returned to employees in 2013
—  more than $5 million in total savings over five years.

SeeChange, a former regional PPO health plan, recently shifted its focus away from insurance and entirely toward analytics, using a variety of data sets from claims, labs, pharmacy and biometric data from wearable devices and self-reported patient data.

JFK Health, a small nonprofit system that includes JFK Medical Center and a number of inpatient and outpatient rehab centers, nursing facilities and specialized treatment, said it sought to quell costs with more active engagement and early detection.

“We realized that the best way for us to begin disease management was with prevention and early identification of disease or precursors to disease,” said Pat Cooke, corporate director of HR for JFK Health.

The health system also said employees themselves are saving money – completing screenings and preventive health measures allows them to save 40 percent on their own premiums each pay period. Enrollment in the program with SeeChange’s FlyWheel has grown by 300 percent, according to JFK Health.

SeeChange says its analytics platform helps detect or predict chronic conditions, such as diabetes and coronary artery disease; upon detection, specific efforts are outlined for individual employees to prevent further health issues, based off of nation guidelines and protocols.
