Welltok, a health IT startup that  seeks to help health plans and population health managers better optimize incentives, raised $12 million from Hearst Heath Ventures, capping off a Series D round that totals $37 million.

All told, the Denver-based startup has now raised $59 million. The company’s lead product is its CaféWell Health Optimization Platform, a web and mobile-based platform that organizes apps and tracking devices, providing what is describes as “personalized guidance and leveraging social, gaming and cognitive technologies.”

In addition to Hearst Health, a division of the Hearst Corporation, Catholic Health Initiatives is a new investor. Joining Hearst and CHI are Bessemer Venture Partners with participation from existing top tier investors, including Emergence Capital Partners, InterWest Partners, New Enterprise Associates and Qualcomm Ventures.

The funds will be used to launch new products and services, as well as expansion into new market segments. CaféWell Concierge is the company’s newest application, powered by IBM Watson.
