A diabetes education program  is seeking to build a support network using physical and video interaction between participants and a patient education network to ensure better outcomes.

Live Health also seeks to increase patient referrals by providing automated care coordination between a primary care provider and diabetes education team.

With so many type 2 diabetes patients uninformed about how best to manage their diets and health, the program’s education team would interact with a patient through a video hookup on the Internet. Group and one-on-one sessions are also part of the program.

Live Health CEO Chris Day said: “We reduce the burden on the patient by providing care wherever and whenever it is most convenient for the patient.”

A gaming component to the program includes an exercise in which participants navigate several dietary challenges such as snacking in the workplace, a business dinner and healthy grocery shopping choices.

The business model revolves around sponsored outreach campaigns and a care module in which it would increase the “insurance pie” by increasing revenue through health incentives.

Day made the pitch as part of his company’s entry in the Sanofi Data Design Diabetes Innovation Challenge.

“The largest challenge faced by people with diabetes is a healthy lifestyle change,” Day said. “Emotional connections make a difference to improve outcomes. We create emotional connections through technology.”

The company’s road map over the next three to four months is to roll out the program with small to midsized providers, followed by a launch of the care and support network, eventually increasing its reach to five to 10 cities.
