In order to accommodate our members, the Kaiser Permanente Lakewood office is relocating to 20575 Center Ridge Road, Rocky River, Ohio 44116, which is conveniently located approximately five miles away from the current location, and is central to the Lakewood-Rocky River-Bay Village-Fairview Park area.
The KPO Rocky River Medial Offices will be open for patients on Monday, April 19, 2010. The Lakewood Medical Offices will close on Wednesday, April 14 at 5 p.m. and we will move from the Lakewood medical offices to the new Rocky River location starting Thursday, April 15. There is free, easily accessible parking in our new location.
Internal Medicine, Family Practice, Pediatrics, Lab, and Pharmacy services will be moving to the new Rocky River location, along with support services such as registration. If Kaiser Permanente members have questions or need more information, they should contact Customer Relations at 1-800-686-7100 or 1-877-676-6677 TTY/TDD, Monday – Thursday, 8:15 a.m. – 5 p.m. and Friday, 9:00 a.m. – 5 p.m.
