Medocity offers a patient-centric ecosystem that provides comprehensive care coordination, empowering patients to take an active role their health, for instance for cancer patients.

Patient receives the following: secure video for telehealth; ONC-certified messaging; condition-specific health questions relevant to level of acuity; symptom-based guidance factored on severity; multimedia customized resources; medication education and tracking; multiple access points to care team; vitals monitoring (Bluetooth-enabled); Nutritional guidance and Social health communities Care provider and is powered by clinician-driven protocols. The technology allows EMR integration and Bluetooth connectivity and is hardware and operating system agnostic.

The Impact
The platform is designed to maximize medication adherence and supports early detection of worsening symptoms to enable proactive intervention. It also leverages patient-reported health data to optimize clinical decision-making and facilitates care transitions utilizing shareable care plans and patient data. Medocity Improves care coordination continuity through the alignment of multidisciplinary healthcare providers.
