Signify Health acquires TAVHealth to boost efforts in social determinants of health

TAVHealth builds networks of community health partners which are linked through the use of a single platform used to share information, coordinate services and keep track of patients as they referred to different organizations.

Dallas, Texas-based Signify Health, which provides complex care management services and specialized at-home medical services for Medicare Advantage patients, has acquired TAVHealth to extend it capabilities in addressing the social determinants of health.

TAVHealth builds networks of community health partners which are linked through the use of a single platform used to share information, coordinate services and keep track of patients as they referred to different organizations. Financial terms of the deal were not disclosed.

Signify Health’s efforts with vulnerable populations often highlight needs outside of clinical care like food insecurity, transportation issues and unstable housing.

The acquisition of TAVHealth will provide Signify a pathway to address some of these social determinants of health within its internal system allowing for better care coordination and consistency in referrals and follow-ups through a system that both clinical care and social records.

“A surprisingly high percentage of health outcomes are driven by social, environmental, and behavioral factors,” Signify Health CEO Kyle Armbrester, said in a statement.

“While we have rich data on these factors from what our clinicians capture while caring for members in the home, healthcare has historically had a last mile problem on connecting the need with the fulfillment. TAVHealth provides the tools and network to put food in the fridge and keep our most vulnerable members safe and secure in their own homes.”

Signify Health was itself the result of a merger between CenseoHealth and Advance Health last year.
