The Product
ChronicCareIQ converts patients’ smartphones into monitoring devices that proactively prompt patients for feedback related to their current health status. The questions and frequency with which they are asked form ‘protocols’ engage patients between 85 percent and 93 percent on a weekly basis. Broadly customizable across populations and co-morbidities and individualized for unique metrics like blood pressure, patient responses are uniquely displayed on a color coded dashboard that enable medical staff to identify poorly trending patients in real-time.

The Impact
It engages patients to become more participatory in their care, has been used to accelerate medication titration, and significantly reduces inbound calls enabling nurses to become more proactive.

Doctors using ChronicCareIQ simultaneously qualify for CMS payment reforms of chronic care management services of $42 per patient per month without having to see additional patients in the office. ChronicCareIQ also automates transitional care management for post-discharge patients. The platform automates audit logs and compliance requirements to minimize risk while maximizing patient care and reducing costs to the healthcare system.  

Customers or Pilots
ChronicCareIQ is being used at 20 active ambulatory clinics with more than 1,000 managed patients and 5 more installations on the implementation calendar. On track to sign with first major health system.

ChronicCareIQ  charge a flat fee per provider per month.
