Valley Health Plan applies AxisPoint Health’s care management solution

Valley Health Plan, a California-based organization with 160,000 members, has implemented AxisPoint Health’s CarePoint condition management solution.

Healthcare is moving toward a more personalized approach to patient care. And it’s not limited to hospitals and health systems — even payers have jumped on the bandwagon.

San Jose, California-based Valley Health Plan is one such organization. As such, it has implemented AxisPoint Health’s new CarePoint condition management solution.

Based in Westminster, Colorado, AxisPoint Health brands itself as a population health management company seeking to achieve the Triple Aim.

Its CarePoint solution utilizes analytics to sort and target specific individuals who need support. On a more specific level, it mines patient data from numerous sources and assigns members to specific care tracks. Whereas care management traditionally focused on five core conditions (asthma, diabetes, heart failure, chronic obstructive pulmonary disease and coronary artery disease), CarePoint takes a broader and more holistic approach as it considers the behavioral and economic factors that impact each patient.

CarePoint then activates a care team that offers patients support and guidance. Additionally, the solution frequently touches base with patients to ensure they’re engaged.

Ultimately, this results in improved quality and lower costs. For one customer, CarePoint led to a 45 percent increase in program engagement over the course of one year, according to AxisPoint Health. Other notable results include a 7.9 percent decrease in hospitalizations and a 12.3 percent average net savings.

Valley Health Plan, which has 160,000 members, chose CarePoint because it can engage with complex populations.

AxisPoint Health launched CarePoint in early June, and Valley Health Plan is its first client to implement it.
